| 一、 | 依據教育部 106 年 5 月 19 日臺教師(二)字第 1060071786 號函辦理。 |
| 二、 | 為利執行旨揭事項業務，教育部委請國立臺灣師範大學(以下簡稱承辦學校)協助建置「高級中等以下學校及幼兒園教師證書核發服務系統」(以下簡稱服務系統，網址： https://certificate.moe.gov.tw)及統一受理高級中等以下學校及幼兒園教師證書發證審查服務(包括：加科登記、另一類科教師、教師證書遺失補發等)。 |
| 三、 | 本次要點修正重點摘錄如下： |
| （一） | 受理教師證書申請補發作業除因證書遺失、更改姓名、性別或國民身分證統一編號、毀損外，增列誤繕之申請補發條件。 |
| （二） | 明定曾取得教育部發給有效之高級中等以下學校及幼兒（稚）園教師證書者、或曾取得臺灣省政府教育廳發給有效之高級中等以下學校及幼稚園教師證書、試用教師證書、偏遠及特殊地區教師證書者，具本要點規定申請資格。 |
| （三） | 增列第 3 點第 6 款，明定依原職業學校技術及專業教師甄審登記遴聘辦法，取得臺灣省政府教育廳發給教師證明書或證書，其補發教師證書名稱為教育部技術及專業教師證明書。 |
| （四） | 增列第 4 點第 6 款，明定教師證書誤繕者，應繳交之文件。 |
| （五） | 修正第 8 點，申請人提供之資料有虛偽不實者，除不予發給證（明）書外，涉及刑事責任者，移送司法機關依法辦理。 |
| （六） | 增列第 9 點，曾取得教育部（或臺灣省政府教育廳）發給高級中等以下學校及幼稚園教師證書者，其教師證書失效者(未曾擔任教職或脫離教學工作連續 10 年以上者 )，得向教育部申請發給曾具教師資格證明。 |
| （七） | 增列第 10 點，已故教師遺族因申請撫卹之需，得向教育部申請發給教師資格證明。 |
| 四、 | 申請教師證書補發時，請確實依旨揭要點規定備妥相關申請表件辦理；另每年 4 月至 8 月為教育部發給教師證書高峰期，請避開此時段之申請作業。 |

基本資訊和雙語連絡方式School Profile 基本資訊Overview 花蓮縣花蓮市忠孝國民小學建校六十年，深耕地方教育有目共睹。創校以來，為社區的國民教學奠下堅實的基礎，培育無數人才，成為社會中堅。本校位於花蓮市中心區域，交通便捷，屬都市型學校。學區內，家庭組織以工商業及軍公教業居多，家長皆重視子女教育。 現任校長是紀忠呈先生（Principal Chi , Chungcheng）。全校教職員總計逾40人。本校小學部共有20班，學生人數約500人；幼兒園2班，學生人數約60人。 Hualien Municipal Jhong Siao Primary School has had 60 years of history in Hualien with a high reputation for our educational focus. For the past few decades, our school has built a solid academic foundation for the community and nurtured innumerable talented children who turn into the backbone of society. The campus is located in the heart of Hualien City with easy accessibility to many places in downtown. Most of the parents of our pupils work in business, industry, military, public, and education sectors, and all of them highly value their children’s education. Principal Chi , Chungcheng is currently the leader of more than 40 staffs and around 500 students in primary level as well as about 60 pupils in kindergarten. 學校發展歷史 School History 本校於1963年創校。1984年全校班級數20班，學生數最高達853人；2002年班級數最高達24班，學生數768人。創校至今60年，合計有10位校長接力經營本校（含現任）。 Jhong Siao Primary School was founded in 1963. The number of students peaked in 1984 with the total of 854 pupils, and the number for classes reached the highest point of 24 in 2002.
